How to turn a transcript into meeting minutes

The transcript is open in one window and an empty minutes document in the other, and most of what is in the first window does not belong in the second.

The quick answer

To turn a transcript into meeting minutes, read it once and keep only four kinds of line: a decision, a task a named person accepted, a question left unanswered, and the context a decision cannot be understood without. Delete the rest. Then check every name, number, amount, and date against the audio before you send it.

01

Read the transcript once and mark four kinds of line

An hour of discussion produces thousands of words of transcript and perhaps three hundred words of minutes. The first pass is deletion. Write nothing yet.

  1. Read from the top without editing. Put one letter beside each line that matters: D for decision, A for action, Q for open question, C for context.
  2. Return to every A and look for three things in the same passage: a named person, a deliverable you could recognise as finished, and a date. Note which of the three is missing.
  3. Delete every unmarked line. Greetings, audio checks, and the exchange about the loading bay door go first.
  4. Rewrite each marked line as one short labelled line, naming the speaker only where ownership or a number depends on it.
  5. Group the four lists under the headings your minutes format already has.

  • 18:52Dana Then we hold the switch until November.Decision
  • 19:10Marco Did anyone ever fix the loading bay door?Left out
  • 19:41Dana I’ll rerun the numbers with the November volumes and send them round by Friday.Action
  • 20:47Marco I can, or Priya can, whoever speaks to them next.Open question
Five lines from the supplier review below, each marked on the first read. The tangent never reaches the minutes.

Transcripts arrive in different shapes. A video call exports numbered caption cues with start and end times on their own lines. Turn cues back into sentences before the first pass, or you will mark the same sentence four times.

02

One transcript excerpt and the minute lines it produced

Below is an excerpt from a supplier review recorded on Tuesday 22 September 2026, 18:04 to 20:47 into the file. Ten lines of speech. Six reach the minutes, and two of the six exist only to explain a third.

Transcript line, 18:04 to 20:47Survives?What it becomes in the minutes
Dana 18:04: “Right, is everyone on? Marco, you’re breaking up a bit.”NoNothing. Connection chatter.
Marco 18:09: “Better now. So, um, the Carrow quote came back at 4,180 a month, which is, what, about nine per cent over what we pay now.”YesContext: the Kerrow quote is 4,180 a month, about nine per cent above the current rate (Marco). The transcript spells the supplier “Carrow”; the audio says Kerrow. Both figures were checked against the recording.
Priya 18:31: “Nine per cent. Yeah. I can’t sign that off before the quarter closes on 31 October.”YesContext: Priya cannot sign off the increase before the quarter closes on 31 October 2026.
Dana 18:52: “Fine. Then we hold the switch until November.”YesDecision: hold the packaging switch until November. Reason: the increase cannot be approved inside this quarter.
Marco 19:10: “Did anyone ever fix the loading bay door? It’s been sticking since August.”NoNothing. A tangent nobody accepted. It goes on the next agenda, not in this record.
Dana 19:41: “Back to packaging. I’ll rerun the numbers with the November volumes and send them round by Friday.”YesAction A1: rerun packaging costs on November volumes and circulate. Owner: Dana. Due: Friday 25 September 2026, resolved from the meeting date and flagged.
Marco 20:02: “We could also just, like, split the order between two suppliers, half and half.”With the next lineNothing on its own. A proposal with no response is not a decision.
Priya 20:15: “Kerrow won’t hold that price under 5,000 units, so splitting loses the discount.”YesRejected option: split the order between two suppliers. Reason: the quoted price needs at least 5,000 units (Priya).
Dana 20:44: “Who’s telling Kerrow we’re waiting?”With the next lineOpens the unresolved item recorded below.
Marco 20:47: “I can, or Priya can, whoever speaks to them next.”YesOpen question: who notifies Kerrow of the delay. Expected from: Marco or Priya. Next step: not agreed.

Read the right-hand column alone and the record still works. The last row is the one people delete under time pressure. Leave it in. “Not agreed” beside a question is what gets it asked again, and a blank there is more use to the reader than a name you picked at your desk.

This page covers what goes into the fields. The blank layout itself, with the meeting header and the correction line, lives on the meeting minutes template page, and these lines drop straight into it.

03

Copy the rules you apply to every line

Transcript to minutes: filtering rules
TRANSCRIPT TO MINUTES: FILTERING RULES

KEEP
1. Anything a named person committed to doing. Record the task, the owner, and the date. Write “To confirm” for whichever of the three is missing.
2. Any option that was rejected, with the reason given. A reader who does not know why will propose it again.
3. Every number, amount, percentage, quantity, and date spoken out loud, with the person who said it.
4. Any question left unanswered, plus who is expected to answer and by when. Where nobody agreed, write “Not agreed”.
5. The one or two sentences of context a decision cannot be understood without.

CUT
6. Greetings, audio checks, apologies for lateness, and sign-offs.
7. Repetition. Where the same point is made three times, keep the clearest one.
8. Ideas nobody responded to, and complaints nobody accepted. Move them to the next agenda.
9. Wording never firmed up: “we could”, “maybe”, “someone should”. Either the transcript resolves it later or it is an open question.
10. Your own explanation of what people meant. If the reason was not spoken, it does not go in.

REWRITE
11. One labelled line per item, no filler words. Label it with its category, then the fields that category carries.
12. Turn relative time into a calendar date. “Friday” and “end of month” become a full date, marked for confirmation.
13. Keep negations and conditions word for word: “only if”, “after”, “not yet”, “unless”.
14. Never merge two speakers into one commitment, and never move a commitment to the nearest name.

BEFORE YOU SEND
15. Re-check every name, number, amount, and date against the audio.
16. Mark the document “Draft, awaiting correction” and name the date you will finalise it.

The rules run in the order you use them. Deletion, then rewriting, then the audio check. Running rule 15 before rule 6 spends your attention on lines you are about to delete.

04

Tell a decision from an action item

CategoryHow to recognise it in the transcriptWhere it goes wrong
DecisionThe group settles a choice and moves on. Plural subject, no commitment attached: “we hold the switch until November”. One voice saying “I think we should” is a proposal until a second person agrees.A proposal recorded as a decision because nobody argued. Silence after one person speaks is not agreement; look for a second voice or the chair closing the topic.
Action itemA named person accepts work in the first person: “I’ll rerun the numbers”, “leave that with me”. Work assigned to someone counts once that person answers. An owner assigned because a name was said nearby. If the named person never answered, it stays an open question.
Open questionThe question is asked and the talk moves on, or two people offer and neither is chosen: “I can, or Priya can”.Quietly resolved by the person writing the minutes. Picking one of the two volunteers at your desk creates an owner who never agreed.
ContextA fact, a figure, or a constraint that makes one of the three above readable to somebody who was not in the room.Padded out with your own reasoning. Only what was spoken goes in, attributed to the person who said it.

Mistakes here run in one direction. A suggestion turns into an action item because a name was nearby in the conversation. Read your action list back and ask, line by line, which words the owner actually said. Where the transcript has none, the line is an open question.

The note Phrase wrote from a café meeting keeps action items, with owners and dates, apart from decisions.

Interview transcripts handle the same problem with a marker: an unresolved passage keeps an [unclear] tag until somebody establishes the words. Minutes borrow the habit. A visible gap beats a tidy field that is wrong.

05

Check names, numbers, amounts, and dates against the audio

A transcript holds what was said, and not much of what was meant by it. Four categories of detail are recoverable from the text itself, and all four are worth checking: names, numbers, amounts, and dates.

Those four are where a transcription error does damage. A misheard name hands work to the wrong person. A dropped digit turns 4,180 into 418. “Thirtieth” and “thirty-first” sound almost identical at speed, and a due date is the field people act on.

  • DateTranscript: 31 OctoberAudio: 31 OctoberMatches
The four checks on the supplier review, each played from its own line: one name corrected, three figures confirmed.

Re-listening to the whole recording is not the method. In Voice Memos on iPhone, open View Transcript and select a word: playback moves to that point, so you hear the sentence around it and move on. Inside a recording in Notes, Find in Transcript does the same job within that one recording, and tapping the transcript text jumps the audio to the matching place. Both need a transcript to exist. Apple limits transcription to iPhone 12 and later, in ten listed languages, and says it is not available in all countries or regions. Checked against Apple’s support pages on 2026-09-20.

Phrase keeps that loop inside one note. A recording or an imported audio file becomes a note with a title, summary, transcript, and organised body once the task finishes, and the original audio stays attached with a timestamped, searchable transcript, so the four checks run against the source. From there the assistant will draft the recap as an email you send yourself, or turn the same note into a one-page brief. Media processing runs in the cloud.

Questions people ask while writing the minutes

How do you write meeting minutes from a recording? Get the transcript first, then apply the rules above to text. Working straight from audio means scrubbing back and forth for every field. With a transcript you delete on one pass, rewrite on a second, and return to the audio for four things.

How to take meeting notes without recording? Keep four running lists live, one per category, and say each decision back to the room before the discussion moves on. Confirming out loud is the part a recording lets you postpone. Leave a field blank rather than filling it from memory an hour later.

Meeting recap email example: open with the single decision that matters, then the action list as owner, task, date, one line each, then the open questions with the date you need answers. Close with a correction deadline. “Corrections to me by Thursday; after that this is the record.”

Meeting minutes vs notes: notes are yours and can stay messy, since you were there. Minutes are the record other people act on and cite months later, so every line carries an owner, a date, or a reason. Once somebody else has to act, write minutes.
